In February 2025, the EU AI Act first enforcement provision took effect. Sixteen months later, most European hospitals are yet to be compliant with it, and by doing so, delaying a clinically AI stronghold.
It is a matter of time for the European Union (EU) to standardize clinically AI literacy across their workforces, while preparing for even stricter 2027-2028 risky compliance deadlines to link innovation and institutional safety.
The requirement in question is AI Literacy – a mandate that organizations deploying automated systems ensure their staff have sufficient technical, and ethical, competence to meaningfully oversee them. By the Act’s own taxonomy, it’s a threshold condition, and without it, every subsequent risky deployment rests on a foundation that regulators can go against.
Most of European medical institutions have not met that threshold, and at a particularly uncomfortable moment medical technology (MedTech) companies are stepping up predictive diagnostic tools’ integration and robotic surgical systems into clinically AI workflows at a speed that Europe’s regulatory preparation has failed to match.
The approach is moving towards data would mean the end of the old ways of handling data and move to a much more structured and patient-centric digital ecosystem. Brussels’ assuming an increased regulatory function, but changeover to the new regime from the existing systems would call for a seamless transition between law and medicine.

Whereas much of the discussion revolving around the EU AI Act seems to center on the technical details related to algorithms, an equally important deadline approaches in regard to the human component of the medical sector. As of February 2025, Article 4 of the AI Act became effective, making AI literacy a mandatory obligation for all organizations.

A visible disconnect exists between the speed of clinically AI adoption and staff training. In high-stakes medical technology, AI integrates into workflows far faster than institutions can build the necessary internal regulation.

By August 2026, when risky clinically AI obligations apply, the medical community must close the literacy gap. Success depends not on the number of tools owned, but on how effectively users understand them.
